The Division of Health records Rahway occurrence births, marriages and deaths. Forms and instructions are available for putting the father’s name on a birth certificate in the event of an out of wedlock birth, and for adding the husband’s name if the parents marry after the birth. Help is available for those persons who are attempting to file a delayed birth certificate.

How to obtain a Birth, Death, or Marriage Certificate

 Applications for birth certificates may be completed between 8:30 am and 4:30 pm.
To obtain a certificate in person, valid picture identification is required or two forms of the following:

Non-photo drivers license Green card
Vehicle registration County ID card
Insurance card School ID
Voter registration card

Utility bill

Passport

To obtain a certificate by mail, please contact the Division of Health at (732) 827-2085

The fee per certified copy for each certificate is as follows:

Birth & death certificate:      $10

Marriage certificate:             $10

Please do not send cash.  We accept checks or money orders made payable to City of Rahway.

Certificates requested in person will be provided immediately.  Mail requests are processed by the next business day or following day after receipt.

Birth Certificates

To request a birth certificate, the following information must be provided:

·                    The exact name of person named on the record (first, middle, last).

·                    The exact place where the birth event occurred (city of birth).

·                    The exact date the birth event occurred (month, day, year).

·                    The mother’s maiden name.

·                    The father’s name (when recorded).

The relationship between the requestor and the person named on the birth record will be the determining factor in deciding whether the Division issues a certified copy of the birth record.  This includes the following:

Subject Siblings
Subject’s parent(s) Legal guardian
Current spouse of subject Legal representative
Child of subject Court order
Grandchild of subject

Married women will need to provide a copy of their marriage license to prove their maiden name when requesting a birth certificate.

 

Death Certificates

To request a death certificate, the following information must be provided:

·                    The exact name as it has been recorded on the death record (first, middle, last).

·                    The exact place where the death event occurred (city of death).

·                    The exact date the death event occurred (month, day, year).

·                    The maiden name of the deceased subject’s mother.

·                    The name of the father of the deceased subject (when recorded).

The relationship between the requestor and the person named on the death record will be the determining factor in deciding whether the Division issues a certified copy of the death record.  This includes the following:

Surviving spouse of subject Siblings
Subject’s parent(s) Legal guardian
Child of subject Legal representative
Grandchild of subject Court order

Marriage Certificates

To request a certified copy of a marriage certificate, the following information must be provided:

·                    The names of the applicants as they have been recorded on the marriage record.

·                    The exact date the marriage event occurred.

·                    The county, township or town where the marriage event occurred.

The relationship between the requestor and the person named on the marriage record will be the determining factor in deciding whether the Division issues a certified copy of the marriage record.  This includes the following:

Applicants

Siblings

Subject’s parent(s) Legal guardian
Children of the subjects Legal representative
Grandchildren on the subjects Court order

OBTAINING A MARRIAGE LICENSE

Who should apply
Males or females who reside in the City of Rahway.  If both applicants are from out-of-state, the application must be filled out in the municipality where the ceremony will occur.
Where to apply
City of Rahway Health Office, Monday thru Friday between 8:30 a.m. and 4 p.m.
When to apply
It is recommended that the license be requested at least four weeks before the ceremony. There is a mandated 72 hour waiting period from the time of application until you can use your marriage license. Weekends are counted in the waiting period.
After the license is issued and picked up, the marriage license may be used in the state of New Jersey for (30) thirty days, after which time the license expires.

APPLICATION REQUIREMENTS

Who should be present at application time

Applicants and one witness 18 years of age or older who speaks English and has valid photo ID.

Applicant(s) must have with them the following items:

  •  Birth certificates (translated if in a foreign language) divorce, annulment or death certificate (if previously married).
  • $28.00 fee
  • Proof of identity by presenting a driver’s license, passport or state/federal/county ID
  • Proof of residency such as a driver’s license, lease, tax return, etc.
  • Social Security card or Social Security number if known

Applicants must know:

  • Both parents’ birth places
  • Both mothers’ maiden names
  • Both fathers’ names
  • Name and address of person officiating the marriage as well as the date the ceremony is to take place.

Blood tests are no longer required to obtain a marriage license in New Jersey
